Caesarean birth as a component of surgical services in low- and middle-income countries.

چکیده
We were very pleased to see further attention drawn to the issue of surgery as a global public health issue in lowand middle-income countries by Ozgediz et al. in the August 2008 edition of the Bulletin.1 We write to draw attention to one component of surgical services, Caesarean birth, which has been well documented relative to other types of surgery. Nationally representative data on Caesarean birth are available for approximately 90% of births in developing countries. Similar results from two separate compilation exercises have been published2,3 for the years around 2000 and efforts are underway to compile data for 2005. Many but not all of these data come from Demographic and Health Surveys, which also allow disaggregation by socioeconomic status.4 Moreover, substantial efforts have gone into determining the unmet need for Caesarean birth by defining indications for Caesarean that are “absolutely” life-threatening.5 Women who experience these problems are unlikely to survive if they do not receive a Caesarean. Decentralization and Regionalization of Surgical Care: A Review of Evidence for the Optimal Distribution of Surgical Services in Low- and Middle-Income Countries
Background While recommendations for the optimal distribution of surgical services in high-income countries (HICs) exist, it is unclear how these translate to resource-limited settings. Given the significant shortage and maldistribution of surgical workforce and infrastructure in many low- and middle-income countries (LMICs), the optimal role of decentralization versus regionalization (ce...

BACKGROUND The Millennium Development Goals call for a 75% reduction in maternal mortality between 1990 and 2015. Skilled birth attendance and emergency obstetric care, including Caesarean section, are two of the most important interventions to reduce maternal mortality. Although international pressure is rising to increase donor assistance for essential health services in developing countries,...

When the Japanese government adopted Western medicine in the late nineteenth century, it left intact the infrastructure of primary care by giving licenses to the existing practitioners and by initially setting the hurdle for entry into medical school low. Public financing of hospitals was kept minimal so that almost all of their revenue came from patient charges. When social health insurance (S...

A recent editorial by Naoki Ikegami has proposed three key lessons from Japan’s experience of achieving virtually universal coverage with primary healthcare services: the need to integrate the existing providers of primary healthcare services into the organised health system; the need to limit government commitments to finance hospital services and the need to empower providers of primary healt...
متن کاملCommunity Health Worker Programs to Improve Healthcare Access and Equity: Are They Only Relevant to Low- and Middle-Income Countries?
Background Community Health Workers (CHWs) are proven to be highly effective in low- and middle-income countries with many examples of successful large-scale programs. There is growing interest in deploying CHW programs in high-income countries to address inequity in healthcare access and outcomes amongst population groups facing disadvantage. This study is the first that examines the scope and...
